In-Home Community Supports and Companion Care
Habilitation Services are delivered in the home and community, with supports tailored to each person’s goals, abilities, and preferences. Services may include:
- Meal planning and preparation
- Money management and budgeting support
- Community integration and outings
- Socialization with peers
- Exercise and fitness activities
- Entertainment and leisure activities
- Recreational and volunteer opportunities
- Health and safety skills
- Personal care and assistance with activities of daily living (ADLs)
- Household chores and home skills
- Assistance with appointments and transportation
- ICHS 2:1 (Level 3) support when needed
Every plan is person-centered, focusing on what matters most to the individual and their family, whether that is learning new skills, staying active, building friendships, or becoming more involved in the community.
ODP Waivers and Funding
Traditional is an approved provider through the Pennsylvania Office of Developmental Programs (ODP) and accepts several waiver programs and funding sources, including:
- Consolidated Waiver
- Personal/Family Directed Support (P/FDS) Waiver
- Community Living Waiver
- Base funding, where available
These programs help eligible individuals receive services in their homes and communities instead of institutional settings, making long-term support more accessible and sustainable for families.
